In addition the office will need the following documents to copy and be kept on file:
- Proof of Residence in the Holgate School District (Utility Bill/Rental Agreement)
- Student's Certified Birth Certificate
- Student's Immunization Record
- Legal Custody/Guardianship Documents (if applicable)
- Ohio School Health Record-Physician's Report (If student has had a physical within the last 12 months, a copy of the physical will be accepted.) This may be turned in on orientation day in August due to students having physicals during the summer months.
